Assumption of Risk

I understand that participation in martial arts, Brazilian Jiu-Jitsu, grappling, self-defense training, fitness activities, and related instruction involves physical exertion, physical contact, and inherent risks.

These risks may include, but are not limited to, bruises, cuts, sprains, strains, falls, throws, joint locks, chokes, accidental contact, concussions, fractures, dislocations, serious injury, permanent injury, paralysis, or death.

I understand that these risks may result from my own actions, the actions of other participants, the actions or instruction of staff, the condition of the facility, or the nature of martial arts training itself.

I voluntarily choose to participate, or allow my minor child to participate, with full knowledge of these risks.

I have read and agree to the Assumption of Risk.

General Release and Liability Waiver

In consideration of being allowed to participate in training, classes, events, open mats, private lessons, or other activities at The War Room School of Jiu-Jitsu / The War Room LLC, I hereby release, waive, discharge, and hold harmless The War Room School of Jiu-Jitsu / The War Room LLC, its owners, instructors, staff, agents, representatives, affiliates, contractors, and fellow students from any and all claims, demands, liabilities, damages, injuries, losses, costs, or causes of action arising out of or related to participation in any activity at the facility.

This release includes claims based on ordinary negligence to the fullest extent permitted by New Mexico law. This waiver does not release claims that cannot legally be waived under applicable law.

I certify that I am physically able to participate, or, if signing for a minor, that my child is physically able to participate in the programs offered.

I have read and agree to the General Release and Liability Waiver.

Emergency Medical Authorization

In the event of an injury, illness, or medical emergency, I authorize The War Room School of Jiu-Jitsu / The War Room LLC to seek emergency medical treatment for me or my minor child if necessary.

I understand that The War Room School of Jiu-Jitsu / The War Room LLC does not provide medical insurance for participants, and I am responsible for any medical expenses incurred.

I have read and agree to the Emergency Medical Authorization.

Video Surveillance Notice

I understand and acknowledge that The War Room School of Jiu-Jitsu / The War Room LLC facility may be monitored by video surveillance for safety and security purposes.

By entering the premises and participating in training or related activities, I consent to being recorded for safety and security purposes.

Media Release

I understand that photos or videos may occasionally be taken during classes, events, promotions, or academy activities.

Gym Rules and Conduct

I agree to follow all academy rules, safety instructions, hygiene expectations, and conduct standards provided by The War Room School of Jiu-Jitsu / The War Room LLC.

I understand that unsafe behavior, failure to follow instructions, misconduct, poor hygiene, harassment, or inappropriate conduct may result in removal from training or the facility.

I understand that academy policies, schedules, rules, and procedures may change at the discretion of The War Room School of Jiu-Jitsu / The War Room LLC.

Governing Law

This agreement shall be governed by and interpreted under the laws of the State of New Mexico. Any legal action arising out of participation shall be brought in the appropriate court located in Bernalillo County, New Mexico.
